When you look for ADUs online, you will probably come up with two types of results. One is a company that sells pre-built structures, and the other company designs, permits, and builds the whole project from the ground up. The ads can look very similar, but they have very different prices. If you are trying to figure out which one is right for your property, you’re not the only one, and honestly it can be a bit confusing.

My name is Buz Artiano, and I started BuildX. Since then we’ve built dozens of ADUs all over Massachusetts, but most of our builds have been around the South Shore and Plymouth County areas. We handle all the design, permitting, and construction ourselves. We work within the state’s by-right ADU regulations and local building code requirements every day. Our real life experience with both the legal and construction side of things is what allows us to explain the real differences between the two types of ADUs.

This article breaks down what prefab ADU companies actually deliver, what design-build ADU builders deliver, and where the gaps show up. The goal is to give you the vocabulary to compare quotes accurately so you can choose the approach that fits your property, your timeline, and your budget.

Quick Answer: A prefab ADU company sells a manufactured box, typically a shell with very basic finishes. A design-build ADU builder handles the entire project from design and permitting, to site work, foundation, utilities, and construction. The biggest difference is that prefab quotes usually don’t include the $100,000 to $200,000 in site preparation, foundation, septic, utility connections, and finish work that you have to have. A design-build quote includes everything you need for a finished, permitted, move-in-ready home.

What Does a Prefab ADU Company Actually Sell?

A prefab ADU company, or modular ADU company, sells a manufactured structure. It’s built in a factory, brought to your property, and set on a foundation. The advertised price, which can be between $99,000 and $150,000, covers JUST the physical box: walls, roof, basic framing, and in some cases, a few interior finishes.

Keep in mind that prefab units arrive with primer-only walls, laminate countertops, MDF trim, and ceiling heights as low as six feet seven inches. The finishes that you need to make it feel like a home are quality flooring, cabinetry, countertops, fixtures, and paint. These things are not usually included in the base price.

The most important thing to remember is that the advertised price almost never includes the work you have to do to make the structure actually livable on your lot. A prefab company delivers a product. Your property still needs everything else.

What Does a Design-Build ADU Builder Handle?

A design-build ADU builder manages every detail of the whole project. That starts with an architectural design that is created just for your lot, what you need the ADU for, and your existing home. The next phases are things like permitting, engineering, site preparation, foundation, framing, all mechanical systems (plumbing, electrical, HVAC), insulation, interior finishes, and final inspections.

When we say a project costs between $250,000 and $375,000 for a completed ADU, that price gives you a home you can move into immediately, not a shell that still needs over $100,000 of additional work.

Design-build ADU companies have one team that is accountable for every phase of the project. You won’t need to hire a separate architect, site contractor, foundation crew, or finishing team. One point of contact, everyone is on the same schedule and the same budget.

How Do the Two Approaches Compare on Cost, Quality, and Process?

The easiest way to tell the difference is to look at the same categories side by side. Here’s a comparison across the three things that matter most when evaluating options.

Cost

Prefab: The advertised price typically ranges from $99,000 to $150,000 for the structure. That price excludes site preparation ($10,000 to $30,000), foundation ($25,000 to $50,000), permits and engineering ($5,000 to $15,000), septic or sewer connections ($20,000 to $50,000), utility hookups ($10,000 to $25,000), and interior finish work. When you add it all up, a $150,000 prefab module becomes $350,000 to $450,000 installed. That is the same or higher, than a custom-built site ADU.

Design-build: The quote includes every single item from the start. At BuildX, our completed ADU projects in Massachusetts usually cost between $250,000 and $375,000 depending on site conditions, septic requirements, and finish selections. There are no surprise invoices after the structure arrives.

Quality

Prefab: Factory-built units are very limited on what they can have because they have to be transported on a truck. Ceiling heights are usually capped at six feet seven inches. The structure is actually made up of two rectangular modules that are later joined together, and where they connect, the shared wall can be eight inches thick, which gives you a lot less space inside. Materials are usually the bare minimum quality. Things like laminate counters, MDF trim, and primer-only walls. The exterior can’t be customized to match your existing home’s roofline, siding, or window style either.

Design-build: A site-built ADU is designed for your specific lot. Ceiling heights, floor plans, and exterior finishes are all customized for you. The structure can match your existing home, so it has the same roofline, siding material, and architectural style so it looks like it belongs on your property rather than just a random building that arrived from a factory. Material selections, from countertops to insulation, are chosen by you during the design phase.

Process

Prefab: You purchase the structure from one company. Then you have to coordinate site work, foundation, permits, utility connections, and finish work yourself or hire all separate contractors. Some prefab companies offer to help support you in the process, but they don’t usually know or have local contractors, architects, or project managers in your area.

Design-build: One company manages absolutely everything. At BuildX, we handle the site survey, architectural design, 3D visualization, permitting, engineering, site prep, construction, and final inspection. You work with one team from start to finish. Once we start the actual construction, we guarantee that it will be finished in 100 days or less.

What Do Real Families Experience When They Compare Both Options?

Pam and Cliff Carden are BuildX clients in Freetown, Massachusetts. They’re building an ADU for their adult daughter, who grew up in town and wants to stay in the community she loves. Before choosing BuildX, the Cardens met with a prefab company from Burlington.

Cliff said that the prefab company didn’t have all the contractors lined up. He said "I think the biggest thing is you have assembled a team, a team that consists of various contractors, building design, architects. It made the process a lot easier."

The Cardens also loved the design process. During their design sessions, we worked with designer Samantha to rotate the entire house orientation when the original spot just couldn’t work on their lot. The 3D design tool allowed the family to see changes live. Cliff said the adjustments on screen were instantaneous, he said "it gave us that vision." Pam added, "You didn’t have to worry about what it’s going to look like. You’re actually seeing it."

We rotate ADU orientations more often than most people think. Lot constraints, setback requirements, septic field locations, and the position of the primary house all affect where and how an ADU can be built. On the Carden property, the original placement did not work with the site conditions, so we worked with Samantha to turn the structure around. You’ll never get that type of design flexibility from a prefab module. A factory box arrives in only one configuration. A site-built ADU is designed around your property’s actual limitations from day one.

Why Do Prefab ADU Prices Look So Much Lower Than Design-Build Prices?

The price gap is not a difference in value, you have to look at what the price includes. A prefab quote covers the shell only. A design-build quote covers the entire project. Comparing a $120,000 prefab quote to a $275,000 design-build quote is definitely not comparing apples to apples. The $120,000 number leaves out everything that turns a delivered box into a legal, livable home.

We’ve had families come to us after spending three months with a prefab company, putting down a deposit, scheduling delivery, and then all of a sudden they get a site-readiness invoice that includes site prep, foundation, permits, utility hookups, and finish work for another $180,000 on top of the original $165,000. That family walked away from their deposit because the total was even higher than a custom-built ADU from BuildX would have cost from the start.

There is also a payment structure difference. With many prefab companies, you pay for the whole thing in full before they will set it on your foundation. That means a six-figure payment not even including any site work. With a design-build contract, payments are typically tied to construction milestones through a loan and the payments are manageable.

What Questions Should You Ask Before Choosing Either Approach?

Whether you are talking to a prefab company or a design-build builder, the right questions will make sure you’re not hit with any nasty surprises. Before signing anything, ask about these things specifically.

First, ask what the quoted price includes and doesn’t include. Request an itemized list that covers site preparation, foundation, permits and engineering, septic or sewer, utility connections, interior finishes, and paint. If the answer is "that’s handled separately," ask who handles it and how much it costs.

Second, ask whether the company has a local team of contractors in your area. Massachusetts has some of the most complex building codes in the country, including the energy code, stretch code, and Title V septic requirements. A company that lists a really large service area on its website may not have project managers, contractors, or permit relationships in your town.

Third, ask about the payment schedule. Find out when money is due relative to construction progress. If you’re asked to pay for a structure in full before it sits on a foundation, it’s important that you understand that risk before committing.

Is Design-Build the Right Fit for Every Family?

No. Design-build is the right approach for families who want to go through the whole process with one team managing every phase all the way up to move in. If you have an experienced general contractor you know and trust, a clear understanding of permitting and site work, and the time to coordinate contractors and trades, a prefab structure combined with your own team might work for your situation.

We also recognize that some families are drawn to how fast a modular ADU can be put in. In reality though, the timeline savings are not as great as the marketing suggests, because site preparation, foundation, permitting, and finish work all still take the same amount of time regardless of where the building is coming from. But if your project conditions are pretty simple, a prefab approach could work for you.
